The Data Administrator processes the personal data provided in the form in the Data Administrator' database pursuant to the Act of 29 August 1997 on personal data protection (consolidated text: Dz. U. 2002. No 101, item 926, as amended) and the Act of 18 July 2002 r. on rendering electronic services (Dz. U. 2002, No 144, item 1204, as amended) for the purposes described in the Privacy Policy.

I hereby declare that I was notified on the right of access to my personal data and its correction and request for its deletion and that all of my personal data was provided by me voluntarily
